ppc4xx: Consolidate PPC4xx SDRAM/DDR/DDR2 defines, part2

This patch now adds a new header file (asm-ppc/ppc4xx-sdram.h) for all
ppc4xx related SDRAM/DDR/DDR2 controller defines.
