The encoding of the sub instruction used to handle CONFIG_SYS_MALLOC_F_LEN
can only accept certain values, and the set of acceptable values differs
between the AArch32 and AArch64 instructions sets. The default value of
CONFIG_SYS_MALLOC_F_LEN works with either ISA. Tegra uses a non-default
value that can only be encoded in the AArch32 ISA. Fix the AArch64 crt0
assembly so it can handle completely arbitrary values.

To be more EABI compliant and as a preparation for building
with clang, use the platform-specific r9 register for gd
instead of r8.

Commit e05e5de7fa made the 2 1st parameters of
ARM's relocate_code() useless since it moved the code handling them to crt0.S.
So, drop these parameters.

The purpose of .globl is to export symbols for ld, not to declare external
symbols.
By the way, use the ENTRY() and ENDPROC() macros to define functions rather than
using .global directly.

Note this is a tree-wide change affecting multiple architectures.
At present we use __bss_start, but mostly __bss_end__. This seems
inconsistent and in a number of places __bss_end is used instead.
Change to use __bss_end for the BSS end symbol throughout U-Boot. This
makes it possible to use the asm-generic/sections.h file on all
archs.

Move all the C runtime setup code from every start.S
in arch/arm into arch/arm/lib/crt0.S. This covers
the code sequence from setting up the initial stack
to calling into board_init_r().
Also, rewrite the C runtime setup and make functions
board_init_*() and relocate_code() behave according to
normal C semantics (no jumping across the C stack any
more, etc).
Some SPL targets had to be touched because they use
start.S explicitly or for some reason; the relevant
maintainers and custodians are cc:ed.
