Make driver local variables and functions static and
remove them from the arch header.

The architecture independent header is moved to drivers/mmc, and the architecture
dependent headers reside in asm/arch-omap3 and asm/arch-omap4

This helps to clean up the include/ directory so that it only contains
non-architecture-specific headers and also matches Linux's directory
layout which many U-Boot developers are already familiar with.
